WyCare Goals

WyCare is a project of the Community Health Council of Wyandotte County (CHCWC), dedicated to enhancing access to healthcare and to improving the health status of uninsured and underinsured residents of Wyandotte County. An outline of WyCare goals, desired outcome, and activities follows.


Goal #1: Build health care provider networks for the under- or uninsured, by strengthening the inter-agency/inter-institutional relationships.

Desired Outcomes

Activities

  • Uninsured patients are assigned a medical home at safety net clinics
  • An improved system for referrals among safety net clinics
  • Implement a shared electronic patient registry to facilitate intake and referrals at Wyandotte County, Ks. safety net clinics
  • Hospital emergency departments assign under and uninsured patients to a medical home at safety net clinics
  • Develop protocols to improve referrals out of emergency departments
  • Electronically link hospital EDs to safety net clinics
  • Broaden scope of community health services available to the under- and uninsured
  • Develop a referral system for safety net clinics to access specialist care
  • Electronically link safety net clinics to specialty care clinics at KUMC
  • Increase the number of applications and enrollments in Medicaid
  • Improve community awareness of affordable health care services
  • Implement processes & procedures whereby Social Rehabilitation Services (SRS) staff, clinic staff, and culturally-based lay health advisors provide information to potential clients for safety net clinic services

Goal #2: Create a method for effective allocation of scarce resources enabling safety net clinics to provide health care to uninsured and underinsured patients.

Desired Outcomes

Activities

  • Establish a reimbursement mechanism that fairly distributes payment for point-of-service providers for the uninsured
  • Convert existing radiology voucher program to electronic system
  • Establish voucher program for pharmacy and lab services

Goal #3: Create a health maintenance model for persons with chronic disease, such that health outcomes will be optimized and inappropriate expenditures (such as those associated with non-emergent care in emergency room settings) will be reduced.

Activities

  • Improve the health status of the under- and uninsured
  • Develop a chronic disease management plan to be adopted at safety net clinics
  • Make information about chronic disease prevention and management available at safety net clinics
